His prescribed medications are phenytoin 300mg daily, clonazepam 6 mg daily and fluoxetine 20 mg daily. It transpire that fluoxetine therapy had been initiated 2 weeks previously. The patient's phenytoin level is found to be 35 mg/l; at the last outpatient clinic visit 4 months ago, it was 18mg/1. what is the proposed mechanism of interaction between fluoxetine and phentyoin and how should it be managed?​
